The natural frequency calculation:

  • Out of 100,000 people: 10 have brain tumours, 99,990 don't
  • Of the 10 with tumours: 6 have persistent headaches
  • Of the 99,990 without: 4,000 have persistent headaches
  • Total with persistent headaches: 4,006
  • P(tumour | persistent headache) = 6/4,006 = 0.15%

Your probability went from 0.01% to 0.15% — a fifteen-fold increase, which sounds dramatic. But 0.15% is still vanishingly small. There's a 99.85% chance your headache is caused by something mundane.

This is why doctors don't order MRI scans for every headache. The base rate is so low that even a symptom strongly associated with the disease barely moves the needle.
